Clay City CUSD #10 Registration News
Registration is Friday, July 17, 2009
Grade School Commons Area
10:00 am - 1:00pm and 3:00 pm - 5:30 pm
For Kindergarten - 12th grade
Registration Packets are available for pick up:
At the Clay City CUSD #10 Unit Office
Monday-Friday 8:00-3:00
daily
Otherwise, packets will be mailed on June 29th
Physical and immunication record must be brought in at registration or by the first day of school or your child will not be allowed to start school
If your child is entering any of the following grades, please note the medical forms needed:
K or new students to the district:
1. Physical examination
2. Immunization record
Polio: 3 or more doses with the last dose after the 4th birthday
Diptheria, Tetanus, Pertussis: 4 or more doses with the last dose after the
4th birthday
Measles, Mumps, Rubella: 2 doses with the first at 12 months of age and the second at least one month later
Hepatitis B: 3 doses
Varicella (Chickenpox): 1 dose or proof of having disease
3. Dental examination
4. Vision examination
5. Birth certificate: copy from the county courthouse in the county which your child was born, this is not the certificate from the hospital
2nd grade- Dental Exam
6th grade-Physical, Dental Exam
9th grade - Physical, Tetanus Vaccination |

1. Be sure that the school office has your correct email address
2. Your password will be emailed to the address you provided at registration.
